Generation of Double Eyelid

Anatomically, an eyelid consists of skin, soft tissue, fat, muscles, and tarsal plate. Once the skin and either of the tarsal plate or the muscle fascia are connected, a crease forms on your eyelids. When you open your eyes the skin under the crease will pull up and form a double eyelid.

Therefore, eyelid surgery forms an artificial crease, which connects the skin under your eyelid crease to either the muscle that helps open the eye or the tarsal plate.

 

Methods Of Creating Double Eyelids

This eyelid surgery methods consist of incision method, non-incision method (buried suture), and partial incision.

What surgery method will work best depends on your eyelid type, the thickness of the eyelid skin, the fat volume under the eye skin, strength of the muscle that opens the eye, etc. Your doctor will consider these factors during the consultation.

#1 | Non-incision method (Buried Suture)

Creates natural-looking big eyes without incision

Non-incision method (buried suture) is a method that ties the inner muscles and inner skin of the eyelid together. It is considered the best method for teenagers and young adults in their 20s, whose skin is elastic. If you have sleepy eyes, ptosis correction surgery should be conducted together since the double eyelid surgery cannot change sleepy eyes on its own.

Non-incision method (Buried suture) is suitable for:
  • Those who have thin eyelids and those who have a low volume of fat in the eyelid area.
  • Those who want to have natural-looking eyes.
  • Those whose skin around their eyes is sufficiently elastic.
  • Those who wish to avoid the incision method.

#2 | Incision Method

An incision is made on the eyelid line to remove some tissue, including skin, muscle, fat and connective tissues. Then, the dermis layer bellow the line is stitched to either fascia or tarsal plate, allowing them to adhere to one another by forming scar tissue. The double eyelid line created in this method is accurate and clear.

The incision method gives you a long-lasting eyelid. However, it takes longer for the swelling to subside after the operation. You will also need a bit more time for your eyelid to look natural.

#3 | Partial Incision Method

The partial incision method has the strengths of both the non-incision method (buried suture) and the incision method. Using this method the surgeon creates a 10mm crease in the middle of the fold using the incision method. However, the creases on both sides are formed using buried suture. 

You can get the natural-looking eyes from the non-incision method (buried suture) as well as the distinct eye shape created by the incision method.

The partial incision method has many benefits. The double eyelid fold can last longer than the ones made by the other methods, leaves no scar tissue, the fat around the eyelid can be removed, and the swelling is relieved quickly. However, this method can’t be applied when there are sagging eyelids.

The partial incision is suitable for:
  • Those who have a large amount of fat or muscle around their eyes.
  • Those whose double eyelid crease came undone after the non-incision method (buried suture).
  • Those who want to remove the fat around their eyelids.
  • Those who want to minimize scarring and swelling.

In and out-fold

The In-fold is the most popular fold type among Koreans. The eyelid starts from inside of the epicanthal fold and it becomes wider along the eye line. The line is invisible near the epicanthal fold and the height and thickness of the double eyelid line are natural-looking. It fits the contours of Asian eyes.

This double eyelid crease occurs among Western people and it makes the eyes look big and beautiful. However, this distinctive double eyelid line has no equivalent among Asian’s eyes. The line starts from outside of the epicanthal fold and it becomes wider along the eyelid line. The entire line is invisible and obvious, so eyes with an Out-fold line look bigger. It fits those who have big eyes.

Is a mixe of In-fold and Out-fold styles. This type of double eyelid makes one’s eyes look bright and charming.
